Every pet is unique, and our Pet Profile form helps us to tailor our services to meet their individual needs. We are committed to ensuring the Lodge is a safe and healthy space for all pets who visit us. With that in mind, our Pet Profile covers several aspects of your pet’s health and behavior, and could take up to 15 minutes to complete. At any point, you can save your progress by clicking the “Save & Continue Later” button at the bottom.

We require a Pet Profile for each individual pet joining us for any service annually.  You do not need to complete a new profile for each service with the Lodge, and we will reach out if your pet’s current profile is older than 1 year. We appreciate you taking the time to complete this form and ensuring our Lodge stays safe for all of our furry friends.
